Abstract

Ayurveda, the ancient science of life, emphasizes Panchakarma therapies for disease management, with Basti Chikitsa being regarded as "half of the total treatment." Among its specialized forms, Uttarbasti is a unique procedure primarily indicated for genitourinary disorders, especially in gynecological conditions. This review explores the principles, classifications, instruments, procedural techniques, and modern applications of Uttarbasti. The therapy, administered through the urinary or vaginal route, plays a vital role in conditions such as infertility, dysmenorrhea, amenorrhea, and uterine disorders. Its mechanism of action is understood through Ayurvedic and modern pharmacokinetic perspectives, highlighting its efficacy in local drug absorption and systemic benefits. The review also discusses its indications, contraindications, and procedural advancements. Uttarbasti, when performed with proper pre- and post-procedural care, emerges as a significant therapeutic intervention in Ayurvedic gynecology, warranting further clinical validation in contemporary healthcare settings.
