Tuberculosis remains one of the deadliest disease in the world. The World Health Organisation (WHO) estimates that each year more than 8 million new cases occur approximately 3 million people die from the disease. Extra pulmonary infection accounts for 40% of all tuberculosis cases.[1] Common sites of extra pulmonary tuberculosis are lymph nodes and abdomen. Musculoskeletal tuberculosis occurs only in 3% of cases.[2]
Primary tuberculous pyomyositis is a rare manifestation of musculoskeletal tuberculosis.[2] It can cause a diagnostic dilemma for a physician and surgeon because of its similar presentation to soft tissue sarcomas, hematomas, and myopathies.[2] We present a case of a 20-year-old immunocompetent gentleman with a L thigh & arm swelling with USG guided aspiration after MRI, CBNAAT was positive for tuberculosis patient was started on anti-tubercular treatment . In a tubercular endemic country, a high index of suspicion is required to diagnose this disease which can be cured completely.
